The CAPC values align with those published in the Center to Advance Palliative Care's "Prescribing Opioids: A Reference Guide" syllabus, adapted from McPherson ML's work "Demystifying Opioid Conversion Calculations: A Guide for Effective Dosing," 2nd ed., by the American Society of Health-System Pharmacists in Bethesda, Maryland (2018). EPEC values are derived from the EPEC-O Participants Handbook from 2005.
